About the job
The 22 political municipalities of the Dielsdorf district with around 90,000 inhabitants have joined together to form the Health Center Dielsdorf Association and operate a regional health center that plays a central role in care provision.
As a regionally anchored association, we offer older people and their relatives individual, competent, and reliable advice on all matters related to aging through our Contact and Information Point. For this important task, we are looking for committed specialists who want to contribute to the quality of life of older people in our region with expertise and empathy.
Advice and mediation of services in inpatient and outpatient areas on behalf of the association municipalities of the Dielsdorf district in the areas of volunteer work, everyday design, finances, housing, legal care, neighborhood help, etc.
Coordination and mediation of care places in the supply region / housing in old age
Conceptual work
Organization of meetings involving professionals and offices (general practitioners, social workers, KESB, etc.)
Representing the Contact and Information Point at events / general public relations
Network maintenance, networking work in the sense of case management
Working in an interprofessional team
Training as a nursing specialist (HF or FH) or equivalent training
Further training and experience in geriatrics, communication, case management desired
Knowledge of service providers in the elderly sector in the Zurich lowlands is an advantage
Strong social skills, communication skills, and empathy
Enjoyment of independent development work
Organizational talent and enthusiasm
