Group Leader Family Care (40-100 %)
Your tasks
Case management and coordination
- Case management of clients where relatives provide care services
- Central contact person for all concerns of caring relatives
- Planning, coordination and evaluation of care processes within the scope of family care
- Ensuring communication and cooperation between relatives, nursing professionals, doctors and other parties involved
- Care controlling and approval of service documentation
Quality and expertise
- Ensuring compliance with internal quality guidelines and legal requirements in the area of family care
- Close cooperation with the family care team leader in the development and implementation of standards and training concepts
- Participation in quality projects, process optimisations and internal training
- Promotion of a professional, evidence-based nursing practice
Training and development
- Organisation and support of training, information events and coaching for caring relatives
- Promotion of independence, confidence and motivation of relatives in nursing practice
- Ongoing evaluation of training needs and further development of offers in cooperation with the family care team leader
You bring with you
- Diploma as a nursing professional HF/FH
- Experience in Spitex and/or family care
- Initial leadership experience is an advantage
- Responsible, independent and structured way of working
- High social, communication and organisational skills
- Strong commitment and quality awareness for client care
- Driving licence category B
