SOCIAL WORKER 80% PERMANENT CONTRACT

Mission

• Participate in the administrative and financial management of people with loss of autonomy.
• Support clients in their procedures to assert their social rights.
• Coordinate local actors involved in the client’s home care project.
• Support clients in their socialisation projects and maintaining social ties.
• Provide psychosocial support to clients and their relatives.
• Inform and guide clients in finding suitable accommodation facilities.
• Collaborate within an interdisciplinary framework and work in a network with partners.

Profile

• Bachelor’s degree in social work (HES-SO or equivalent), minimum 2 years’ experience preferred.
• Motivated by home care and interested in elderly people and those with mental health issues.
• Proven knowledge of the Swiss social insurance system.
• Dynamic person, able to work rationally and autonomously within a multidisciplinary team.
• Good listening and negotiation skills.
• Holder of a valid driving licence.
• Comfortable with IT tools (patient file management, care planning, quality indicator monitoring).
• For foreign diplomas, Sefri recognition is required or must be in process.
