Job Description
- Perform screening activities
- Schedule appointments with family doctors, specialists and/or therapists
- Respond to patients' questions
- Prepare patient files, updating them according to their care pathway
- Develop individual health plans in line with the medical record, using a comprehensive approach focused on prevention and management of complex cases
- Collaborate with doctors to personalize and adapt each patient's health plan
Direct, coordinate and monitor users in collaboration with family doctors and the Care Network
Qualifications
Essential requirements:
- Recognized nursing diploma with experience in hospital and home care
- Clinical knowledge
- Knowledge of standard computer tools
- Professional awareness
- Communication and interpersonal skills
- Ability to work in a team
- Organizational and coordination skills
- Ability to create a priority scale
- Autonomy in role responsibilities
- Excellent knowledge of the Italian language
Preferred requirements:
- Knowledge of at least one other national language. Preferably French
