Your Tasks

You ensure individual, holistic, and resident-oriented care and support that focuses on the quality of life of the residents.
You actively support the residents in maintaining and promoting their existing abilities and involve them in decisions regarding their care.
You maintain an empathetic and transparent exchange with relatives and reference persons and are available as a competent contact person.
You independently carry out needs assessments according to the RAIHC guidelines (with home care experience) and create the corresponding care and action plans.

What You Bring

A completed education as a dipl. nurse HF / FH (AKP, DNII, DNI, PsyKP) or an equivalent qualification recognized in Switzerland
You provide professional, needs-based care and support in complex care situations and have solid knowledge in the field of home care and the application of RAI HC
You have a big heart for elderly people and value appreciative, empathetic, and resident-centered care
Very good German skills are essential for communication with our residents and within the team
