About the job
What you achieve
- You are responsible for the comprehensive and individual management of the patient process from admission to discharge through professional support of the patients and their system. Patient-related issues as well as economic aspects are taken into account. The patient coach is the central contact person for all interfaces and coordinates them.
- You conduct consultations and clarifications in psychosocial, legal, and financial situations related to the hospital stay.
- As a patient coach / certified social worker, you advise patients, relatives, and the interprofessional team in planning individually tailored and timely organised follow-up solutions.
- You organise, lead, and record interprofessional discussions and roundtable talks with relatives and aftercare providers.
- You advise and support the medical staff regarding adult protection measures.
- You actively help to further develop the patient coaching area.
What distinguishes you
- You have a completed degree in Social Work (FH / BSc.)
- Ideally, you bring experience in clinical social work and/or knowledge in the medical or nursing field and are interested in becoming a practical trainer.
- You are willing to expand your skills in one of our private hospitals and deepen your expertise.
- You have a quick grasp to recognise difficult situations in time and can prioritise accordingly.
- You enjoy contact with people and approach them with respect and appreciation.
- You like working in a team and advocate for the interests of patients within the interprofessional team.
- You like to actively contribute to the further development of your area of responsibility.
