Type of Work

In this versatile role, you will take responsibility as a competent specialist for case management and care planning of clients in their home environment. You coordinate nursing and care services, conduct needs assessments, and together with your team ensure professional and needs-based care.
You will work closely with various internal and external contacts and ensure smooth information exchange. As a communicative personality, interdisciplinary collaboration comes easily to you and you actively contribute to the high quality of services.

Requirements Profile

To be successful in this exciting role, you have completed training as a Certified Nursing Specialist HF/FH or hold an equivalent qualification.
Ideally, you have already gained professional experience in outpatient care and have an interest or experience in case management. You work independently, responsibly, and in an organised manner. In addition, you are characterised by high professional and social competence as well as enjoyment in working with different stakeholders.
A category B driving licence is required for this role.
