Nurse (m/f) at 60% bilingual FR-DE

Your tasks

Conducting needs assessments with various clients
Nursing care and health checks
Coordinating interventions with the planning team
Regular control visits to the client
Supervising and following up with auxiliary staff
Transmitting information to various stakeholders, updating files
Interdisciplinary cooperation with health insurance funds, doctors, hospitals
Participation in conferences
Regular on-call service

Your profile

Nursing diploma HES or equivalent with recognition from the Red Cross and right to practice
Experience in home care and knowledge of InterRAI HC
Proficiency in IT and mobile tools
Ability to organize independently and flexibility
Sense of responsibility
Fluency in French and German
Willing to provide on-call service some evenings and weekends
Driver's license and residence in the canton of Fribourg
