Introduction
On behalf of our client, a friendly, renowned and rapidly growing Spitex organisation (PA model) with a pleasant corporate culture, we are looking for an experienced registered nurse HF/FH for coordination and case management (without nursing duties) to expand the team. You can strengthen the team either in the city of Bern or in the city of Zurich (workplace selectable, both central locations, parking available).
Type of Work
In this versatile role, you are part of a collegial, dynamic team and take on case management and care planning as a competent professional for the care including preventive support of clients at home. You plan the care including needs assessment, target setting and deployment planning for the FaGes and caregiving relatives (PA) who care for the patients on site. You take over case management and coordination with health insurance companies, advise the teams and support client counselling (by phone, occasionally also on site - e.g. for status assessments).
Together with your team, you ensure optimal delivery of care services and client support. As a communicative personality, you also find it easy to actively collaborate and exchange information with various contacts (management, FaGes, care team, external bodies such as health insurers or authorities). Requirements Profile
To be successful in this exciting job, you have completed the registered nurse HF/FH qualification or an equivalent further education (or are in training for it). In addition, you have already gained diverse practical experience in case management and coordination in the care sector (ideally including Spitex). You enjoy communicating with a wide range of contacts, accompanying care cases with your teams and building a professional relationship based on trust. You work with a high degree of independence, are well organised and have great empathy as well as a healthy sense of humour. Do you also have a driving licence (company car available for occasional visits on site) and consider yourself an upbeat, flexible and team-oriented personality? Then we should meet soon.
