Polyclinic, emergency, outpatient care, hospitalisations: from birth to old age, the Morges Hospital offers a wide range of care services. The establishment has 240 acute care beds, a maternity ward with around 1,500 births per year, and nearly 30,000 emergency consultations. In total, more than 40 medical disciplines have been practised within its walls for over 150 years. Surrounded by vineyards, with an incredible view of the mountains and the lake, Morges Hospital welcomes you in a warm and dynamic setting.
We are looking for the Morges Hospital for a:
Care Coordinator for Chronic Patients and Complex Situations M/F at 80% to 100%
YOUR MISSION
In collaboration with family doctors, you autonomously ensure the follow-up and coordination of patients with chronic diseases or presenting complex medico-psycho-social situations.
Thanks to your organisational skills and initiative, you anticipate patients’ needs and actively contribute to improving their care pathway. You play a central role in organising this pathway to guarantee continuity, quality, and economic efficiency of services.

YOU HAVE
- A recognised CRS nursing diploma or a federal certificate as Coordinator in outpatient medicine with a clinical orientation.
- The ability to juggle multiple interlocutors and situations while maintaining a patient-centred approach.
- An excellent sense of organisation allowing effective prioritisation and management of a large patient portfolio.
- The ability to work autonomously while implementing an interprofessional approach.
- Good command of IT tools and clinical documentation.
- Good knowledge of the health network and medico-social actors.
- Excellent interpersonal and communication skills.
YOUR OBJECTIVES
- To autonomously ensure, in collaboration with family doctors, the coordination and support of patients with chronic diseases or presenting complex medico-psycho-social situations.
- To assess patients’ needs and identify medical, psychosocial, organisational, and economic issues.
- To coordinate interventions between the various professionals involved (doctors, nurses, therapists, social services, caregivers, etc.).
- To ensure regular follow-up of patients, anticipate risk situations, and facilitate access to appropriate resources and services.
- To develop a care partnership with patients to strengthen their autonomy, active participation in decisions, and adherence to treatments and recommendations.
- To participate in the continuous improvement of processes, the quality of care pathways, and ensure rigorous documentation of interventions.
