Your field of activity

• You accompany patients of all ages and life situations as well as their environment with professional competence and empathy through challenging phases
• You provide orientation by advising on social, personal, financial and legal questions in the context of hospital stays and discharge planning
• You develop viable follow-up solutions, provide targeted material assistance if necessary and network with the right specialist agencies
• You are part of an interdisciplinary team and actively contribute to ensuring that transitions are seamless and patient-centred

Your experience

• You have a degree in Social Work or are about to complete one
• You think in a networked way, recognise connections and keep processes in view
• You work independently, solution-oriented and take responsibility
• You contribute with your positive, open attitude to a humorous team where cohesion and mutual support are felt daily
