Spitex Braun in Waldkirch was founded with the vision of enabling a 62-year-old man, who lives with aphasia following a brain haemorrhage, to lead a fulfilling life in his own home. Individual and heartfelt care, as desired by the relatives, is central. Our team consists of five dedicated nursing professionals who care for and support the client daily. In this family-like atmosphere, you can fully contribute your expertise and personality and directly enhance his quality of life.
We are looking for a case-leading nursing specialist from 01.12.26 or by arrangement, with a workload of 20% to 40%.
Your tasks:
• Case management: You take over the evaluation, planning, coordination, and responsibility of the entire nursing process and ensure a smooth workflow.
• Nursing documentation: You are responsible for transparent and comprehensible documentation of all nursing measures.
• Resource- and goal-oriented care: You provide care and support to the client, focusing on his individual resources and setting clear nursing goals.
• Care and support in daily life: You assist our clients in everyday life and promote their independence.
What we are looking for:
• You have completed nursing training at tertiary level (Dipl. Nursing Specialist HF/FH or higher).
• You are familiar with InterRAI Home Care or willing to undergo training.
• Knowledge of the Nexus Spitex software is a plus.
• You are characterised by an independent, precise, and efficient working style as well as a high sense of responsibility.
• Your strong social skills, open and communicative personality, and team spirit complete your profile perfectly.
