Spitex Braun in Waldkirch was founded with the vision to enable a 62-year-old man, who lives with aphasia following a brain haemorrhage, to lead a fulfilling life in his own four walls. Individual and heartfelt care, as desired by the relatives, is at the centre. Our team consists of five dedicated nursing professionals who care for and support the client daily. In this family atmosphere, you can fully contribute your expertise and personality and directly enhance his quality of life.

We are looking for a case-leading nursing specialist from 01.12.26 or by arrangement, with a workload of 20% to 40%.

Your tasks:

• Case management: You take over the evaluation, planning, coordination as well as the responsibility of the entire nursing process and ensure a smooth workflow.
• Nursing documentation: You are responsible for transparent and comprehensible documentation of all nursing measures.
• Resource- and goal-oriented care: You take care of and support the client. The focus is on his individual resources and setting clear nursing goals.
• Care and support in everyday life: You assist our clients in daily life and promote their independence.

What we are looking for:

• You have completed nursing training at tertiary level (Dipl. Nurse HF/FH or higher).
• You are familiar with InterRAI Home Care or are willing to complete training.
• Knowledge of the Nexus Spitex software is a plus.
• You are characterised by an independent, precise and efficient way of working as well as a high sense of responsibility.
• Your strong social skills, open, communicative personality and team spirit ideally complete your profile.
