Social Worker within the Addiction Medicine Service

The Addiction Medicine Service (SMA) of the psychiatric department of the CHUV is looking for a social worker for its Intensive and Community Care Unit in Yverdon-les-Bains, starting from 1 November 2026 at a 50% rate.

Context

The Lausanne University Hospital (CHUV) is one of the 5 Swiss university hospitals. Thanks to its collaboration with the Faculty of Biology and Medicine of the University of Lausanne and EPFL, the CHUV plays a leading role in medical care, medical research and education.

The Addiction Medicine Service offers inpatient and outpatient treatments. Addiction is a complex disease involving biological, psychological and social factors. Effective treatment takes all these factors into account and requires a bio-psycho-social approach.

The Addiction Medicine Service (SMA) also conducts training and research activities. The Intensive and Community Care Unit offers a mobile and multidisciplinary approach to managing addiction disorders with or without substances.

Mission

• You support people with substance use disorders for whom traditional outpatient care is insufficient, by offering care within the community and ensuring close coordination of their care
• You provide your expertise to the professional teams of the SMA and network partners to support them in assisting people with unstable or precarious clinical and social situations
• You intervene directly in the living environments of people, notably in low-threshold centres and on the street, to maintain contact with those who are disconnected and to facilitate their referral to specialised addiction networks
• You analyse the social situation of the people supported and identify their needs in order to propose measures adapted to their situation
• You develop close collaboration with various care network partners, notably SCTP, RI, AI and medico-social institutions, to promote coherent and coordinated care
• You support and coordinate procedures related to housing projects, notably within the framework of DCISA (Cantonal Indication and Follow-up System for Addictions) and CCICP.

Profile

• You hold a Bachelor HES-SO in Social Work or an equivalent qualification. For foreign diplomas, SEFRI recognition is mandatory
• You have a strong interest in the fields of addiction and mental health
• You have experience in street work and community work, as well as a genuine interest in these intervention contexts
• You master the main Swiss social insurances and are comfortable with related procedures
• You are well acquainted with the Vaud medico-social network as well as administrative and legal frameworks
• You are also familiar with platforms such as DCISA and CCICP
• You know how to organise your work autonomously, while demonstrating creativity, responsiveness and initiative
• You easily establish trusting relationships and communicate confidently with various interlocutors. You also appreciate interdisciplinary work
• You prioritise mutual support and collaboration, while showing open-mindedness and flexibility in different situations encountered
• You hold a driving licence.

Depending on profile, the candidate's salary class will be between level 8 and level 9.
