• Personnel, professional and economic management as well as further development of Social Services and Case Management
• Ensuring and further developing psychosocial counselling and support for patients and their relatives in complex social and personal situations
• Professional guidance and support in social insurance law issues as well as ensuring needs-based and sustainable follow-up solutions.
• Professional support and advice to the team on complex issues and challenging case situations
• Strategic and organisational further development of the area including processes and key figures
• Establishment and implementation of an interprofessional, early-starting discharge management, from hospital admission to secured follow-up care.
• Active design and further development of interfaces with clinics, nursing, medical staff and capacity management
• Maintenance and expansion of the network with general practitioners, Spitex, nursing institutions, rehabilitation clinics, authorities and cost bearers
• Development and establishment of cooperations and cross-sector care pathways
• Advancing digitalisation and introduction of suitable digital solutions for referral, case coordination and transfer

Bring

• Degree in Social Work (FH/HF), Nursing or a comparable qualification, further training in Case Management and leadership
• Several years of experience in managing a Social Services or Case Management department, ideally in an acute hospital
• Extensive experience in psychosocial counselling and support of patients as well as in complex social work and social insurance law issues
• Sound knowledge of the social insurance system and the regional care landscape
• Experience in organisational development and change projects
• Strong network and interface thinking as well as negotiation skills
