The Case Management at Hirslanden Bern operates at three locations: Salem Hospital, Beau-Site Clinic, and for the Permanence Clinic. The goal is to organise a suitable, timely inpatient follow-up solution for our patients, within the framework of rehabilitation, spa stays, or nursing home admissions. We rely on close communication between doctors, nursing staff, and relatives. Our focus is on optimal patient care while simultaneously controlling hospital stay costs. YOUR TASKS
- You organise timely, suitable inpatient follow-up or transitional solutions for our patients
- You advise and support patients and their relatives throughout the entire discharge management process
- You coordinate the transfer of patients to other institutions together with internal and external parties
- You work constructively with interdisciplinary teams
- You actively contribute to continuously optimising collaboration with our external partners (social insurance, various clinics, nursing homes, child and adult protection authorities, etc.)
- By supporting the DRG team, you have a data basis to make our discharge management as efficient as possible
YOUR PROFILE
- You have completed basic medical or nursing training and have professional experience in case management – ideally complemented by further training in case management
- You are characterised by a sense of responsibility and an independent, flexible, and loyal personality with strong organisational skills
- Your solution-oriented as well as networked thinking and acting enable you to understand contexts and respond accordingly
- Thanks to your excellent social skills and communication abilities, you find it easy to interact with various stakeholders
- Fluent German skills are required, and any additional language is an advantage
- You are proficient in Microsoft Office
