Dipl. Nursing Specialist HF with Additional RAI Responsibility
Your Tasks
- Professional and individual care and support of residents based on the conceptual foundation of the model of promotive process care
- Planning, implementation and evaluation of nursing measures in compliance with the criteria of the nursing process and involving the affected residents and their relatives
- Co-designing optimal interdisciplinary collaboration with all services involved in the nursing and care process
- Assuming daily responsibility
- Careful and complete maintenance of nursing documentation
- Accurate recording of nursing and care services as well as used nursing and consumable materials. RAI responsibility:
- Conducting and ensuring timely RAI assessments according to legal and operational requirements.
- Responsibility for the correct classification of care needs and the care rating of residents.
- Creation, review and ongoing updating of individual care plans based on assessment results.
- Ensuring complete, professional and comprehensible nursing documentation.
- Advising and supporting nursing specialists in the application of the RAI system and care planning.
- Monitoring assessment deadlines and ensuring data quality and completeness of documentation.
Your Profile
- Completed professional training as a qualified nursing specialist (HF, FH, AKP, DNII, DNI, PSYKP) or equivalent diploma recognised by the SRK
- Experience in long-term care and working with older people
- Experience with the RAI system
- User knowledge of the CareCoach software (nursing documentation) is an advantage
- Empathetic and resilient personality with a high level of commitment
- Very good written and spoken German skills
